Say we are in one of the Cities with a high-ish Doom (6 or 7).
I am fighting in a back ally. My opponent attacks me using the basic attack action:
If I don't do anything do they have to beat the doom (which will be likely higher than my paltry defence of d6 or d4).
Do they auto-hit if I decide not to try and defend myself.
If either of the above, if we are in a full on fight, can I drop my defense and let them roll against doom, or if I am in a manouver am I required to use my printed Defense.
This came up in a one-shot. I let the player just stand there (as we were not in combat) and try his luck. However, now that I am running a long term game I need to think about long term abuse. and what is good for the gander is good for the Goose (NPCs).
Thoughts?

Equally I would make the defender roll against the high doom not to get hit.
Rolling verses doom is to achieve against the world. If you are trying to achieve against another person you roll their defence, be it charisma in social, reason v stealth or against their defence from their manoeuvre.
You do not have to roll against doom to peel an orange or to hammer in a nail. If someone does not/can not defend then its an auto hit probably an auto critical.

Yeah, definitely a hit if the other person is just standing there. I'd use the Coup de Grace rule (at the end of the combat chapter, though I don't have my book in front of me to give you the page).
